Test One: Paragraph Presentation
After brief instructions, the computer visually and aurally presents three short paragraphs. The patient should pay attention to all three, because Test Eight consists of five questions concerning each story. Always use Set One for the initial patient assessment, Set Two the second time, and continuously alternate the two sets for all subsequent testing of that patient. There is no scoring on this test, but circle the number on the score sheet to show which set of stories you used.
Test Two: Filled Interval Lexical Recognition and Recall
This test consists of three trials. After you click the button numbered "ONE," the patient hears and sees the following phrase: "You will have to remember this item:". The lexical item appears in large type centered on the screen, and the computer asks the patient to name the item. If the patient correctly identifies the item, place a check mark on the line next to the item under the column heading "Recognition." If the patient fails to identify the item correctly, place a "-" on the line instead.
Next, the computer shows and states: "Say the names of these items as fast as you can." The patient must then read aloud the intervening lexical items, which appear serially at one per second. Keep track of the number of items the patient misses, either by counting silently or by making hash marks under the column heading "Missed items."
After seventeen intervening lexical items, the patient sees and is asked: "What was the item you were to recall?" If the patient correctly recalls the item, place a check mark on the line next to the item under the column heading "Recall." If the patient fails to recall the item correctly, place a "-" on the line. The next two trials are exactly the same, except the word changes each time.
Test Three: Famous Face Recognition and Naming
After hearing the instructions given by the computer, verify that the patient understands the three possible ways to respond to each picture: (1) give the name of the face, (2) say "yes" for face recognition without the name, or (3) say "no" for unfamiliar faces. When scoring, use check marks for correct recognition and recall and "-" marks for incorrect recognition and recall. If the patient recalls a face but gives an incorrect name (an intrusion), give recognition credit, but write the name she or he gave on the line under the column heading "Name".
Test Four: Filled Interval Object Recognition and Recall
This test follows the same procedure as Test Two, except that it uses pictures instead of words. Administer and record it in the same fashion as Test Two.
