Test Five: Object Recognition

This test consists of three trials; one at 1.00 sec, then 0.75 sec, then 0.50 sec

. Make certain that both you and the patient are prepared to start, because it goes quickly. If the patient correctly identifies an object, place a check mark on the line corresponding to that object. If the patient fails to identify an object correctly, place a "-" on the line instead. It is important to score each picture as it appears on the screen.

Test Six: Lexical Recall

This test consists of four trials of ten words each. Instruct the patient to read the words aloud as they are presented on the screen (1/s). As the patient reads the words, record each word, or at least the first several letters of the word, on the lines provided while glancing at the screen to verify the patient's response as correct or incorrect recognition. If the patient fails to recognize the word correctly, circle the number of the word. The words present randomly every time, so you need to write them down as they appear. After each list, the computer will prompt the patient with the words "Please Recall." Place a check mark after each word that the patient correctly recalls from that list. If a patient recalls a word that was not on that list, or was from a previously presented list, write that word at the bottom of the list.

Test Seven: Object Paired Associates

This test consists of two trials of six pairs. Instruct the patient to name both objects as they appear together. Click the "1A" button ("2A" for the second trial) and record the name the patient gives for each object. During the second half of the test ("1B" and "2B"), the patient will see the first object and must give the second object of the pair. If the patient answers correctly (i.e., can identify the "match"), place a check mark on the proper line. If she or he recalls an incorrect picture or does not remember the picture, write down the incorrect response or draw a dash under the "Recall" column.

Test Eight: Paragraph Gist, Inferences, and Details

This test asks five questions about each of the three stories presented in Test One. When prompted by the computer, use the set (1 or 2) corresponding to the story set presented in Test One. The patient must answer the questions in the order that they appear on the screen. Write the patient's responses on the lines provided, using "DR" for "I don't remember" or "DK" for "I don't know," and click the button to continue to the next question. The test administrator must remain neutral during this test. If necessary, remind the patient that you can neither give any hints, nor confirm/negate any of the answers given. If the patient suddenly remembers the story, do not ask the specific questions again, but instead ask "Can you tell me anything else about the story?".
